Our Pet Lodge offers a secure, enriching home away from home for dogs and cats. Completing our secure Pet Boarding Form before your pet’s stay ensures we have all the details we need to provide personalized care and keep your pet safe and happy.
For All Boarding Guests
We need this form on file for every boarding stay to:
- Confirm your pet’s care instructions and feeding schedule
- Document medications and special needs
- Collect emergency contact information
For Pet Lodge Clients (Boarding, Daycare, Training)
To protect all guests in our Pet Lodge, boarding pets must meet our Pet Health Requirements, which include:
- Current core vaccinations (dogs: rabies, DHPP, leptospirosis, Bordetella; cats: rabies, feline leukemia, core vaccines)
- Be on approved flea/tick prevention year-round
- Have a negative fecal test within the last 6 months
- Have a fecal Keyscreen at least once every 12 months
