AGREEMENT: All pets are cared for by Hueytown Veterinary Clinic & Pet Lodge, its owners, staff or volunteers without liability on their part for loss or damage from disease, death, illness, injury, running away, theft, fire, injury to persons, my pets or to other pets, or property by said pet, or other unavoidable causes, due diligence and care have been exercised. I understand that I am solely financially responsible for any damage or harm caused by my pet(s) while under the care of Hueytown Veterinary Clinic & Pet Lodge. I agree that in admitting my pet(s) to Hueytown Veterinary Clinic & Pet Lodge, they have relied on my representation that my pet(s) is/are in good health and has/have not harmed or shown aggression or threatening behavior toward any person or any other pet. If my pet becomes injured or ill, Hueytown Veterinary Clinic & Pet Lodge shall have the right to diagnose and treat my pet within their discretion and judgment, and such expenses, being reasonable in amount, shall be paid promptly by me, the owner of the pet. If my pet cannot safely adapt to the group daycare, Hueytown Veterinary Clinic & Pet Lodge shall have the right to place my pet in a traditional kennel.
Drop Off and Pick-Up Times: You may drop your pet off as early as 6 AM Monday-Friday and pick up by 7:30 PM. There will be no daycare on Saturdays or Sundays. Pets may still be dropped off and picked up for regular boarding on the weekends. Saturday hours for drop off and pick-up are between 6 AM – 12 Noon. Sunday hours are from 2 PM - 6 PM for boarding drop-off and pick-up only.
Fees for Paying Per Visit: I understand and agree that I am responsible for payment of all fees associated with my pet’s stay at Hueytown Veterinary Clinic & Pet Lodge, as well as fees for urgent medical care as described in this agreement, per visit. Paying fees are due on the day of the visit.
Vaccines: Your pet must be current on all vaccinations. If your pet has not been properly vaccinated, they will be provided at the owner’s expense.
Preventatives: Your pet must be clear of fleas/ticks. If we discover any visible sign of fleas and/or ticks, your pet will be given the proper flea/tick treatment at the owner’s expense. There are no exceptions to
this rule. Your pet must be current on an approved heartworm preventative for the prevention of heartworms and intestinal parasites. Your pet must also have had a negative fecal exam for intestinal parasites within the last 6 months. This will be required for all pets lodging with us or participating in group play to prevent the spread of intestinal parasites. If your pet has not had a fecal exam in the last 6 months they will receive one and appropriate treatment will be provided at owner’s expense for any pet testing positive for intestinal parasites.
Potential Risks at Daycare: When dogs are playing, chasing balls, or roughhousing, there will always be the chance of a dog getting scratched, bitten, or sustaining puncture wounds from another dog. There is no way around this when dogs are running free and playing together. The dogs will also get wet from dog slobber and then dirty from the wetness. We do the best we can to keep all dogs safe and clean while playing. We will always report to an owner if there is an obvious incident during play. If we miss an incident and you notice a mark on your pet, please let us know. There is little we can do to stop every incident, but we can observe closely who your dog is playing with most to watch for any future problems.
Nails: Long toenails may cause injury and/or cause damage to our flooring. You will be notified if your pet’s nails are too long. If at your next visit the nails have not been trimmed, Hueytown Veterinary Clinic & Pet Lodge will trim them at the owner’s expense.
Behavior: Any behavior deemed dangerous or inappropriate by Hueytown Veterinary Clinic & Pet Lodge may result in separation of your dog from the group. The dog can still get activity, just separate from the group, and may have to spend more time in the kennel than the dogs that play well together.
Abandonment: Should you not retrieve your pet within fourteen (14) days of the predetermined pick-up date, and have not conveyed to Hueytown Veterinary Clinic & Pet Lodge that you wish to extend the stay, Hueytown Veterinary Clinic & Pet Lodge will notify you by registered mail at the address provided on medical record giving notice that on the twenty-first (21st) day following the original pick-up date, your dog will be considered abandoned, and Hueytown Veterinary Clinic & Pet Lodge will deliver it to an appropriate rescue group or adopt the pet out to an appropriate home.
Photo/Video: If your pet is boarding or participates in daycare/playtime, you are automatically giving us your consent to display or post pictures and/or videos to our website and social media outlets as we see fit.
I hereby grant Hueytown Veterinary Clinic & Pet Lodge permission to act on my behalf and in my pet’s best interest by obtaining veterinary care at my expense, if deemed necessary, for illness or injury. I further agree to pay for all vet and other necessary services incurred by and for my pet. Hueytown Veterinary Clinic & Pet Lodge staff, in its sole discretion, reserves the right to (a) treat such injuries on site, or (b) have the pet seen by a vet if it is deemed that such injury or illness requires vet care. We will also contact you as soon as possible, but we will not hold up care at any time.
